Insurance

Paradise Psychology, PLLC is currently in-network with the following commercial plans:

  • Optum / United Healthcare

  • BlueCross BlueShield

  • Cigna / Evernorth

  • Aetna

Depending on your current insurance coverage, your services may be partially or fully covered. I attempt to verify your eligibility and benefits with your insurance plan as a courtesy; however, I strongly recommend contacting your insurance plan directly to verify your eligibility and benefits prior to receiving services. A Superbill may be provided for out-of-network billing; however, this does not guarantee coverage or reimbursement by your insurance plan. I recommend asking the following questions when contacting your insurance provider to help determine your eligibility and benefits:

  1. Does my health plan include behavioral health benefits?

  2. Do I have a deductible? If yes - does it apply? If so, what is it and have I met it yet?

  3. Do I have a copay or coinsurance? If so, what is it?

  4. Is prior authorization required for routine outpatient services?

  5. Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  6. Do I need a written referral from my primary care physician in order for services to be covered?

  7. Is my preferred mental health provider in-network? If so, what are my in-network benefits? If not, what are my out-of-network benefits?

  8. (If applicable.) Are telehealth sessions covered under my plan?

Payment

I accept exact amount cash, HSA, and all major credit cards. Payment is expected at the time services are rendered. Electronic billing methods are used for your convenience.

Cancellation Policy

If you are unable to attend your scheduled appointment, please cancel/reschedule at least 48 hours beforehand. Otherwise, you may be charged the full rate of the session. I understand emergencies do happen, and encourage reaching out when unforeseen circumstances arise.

NO SURPRISES ACT

You will have the right to receive a “Good Faith Estimate” outlining how much your medical care will cost.

As of January 1, 2022, federal law requires healthcare providers to give patients who do not have health insurance or who are not using health insurance an estimated bill for medical items and services.

  • You have the right to receive a “Good Faith Estimate” for the total expected cost of any non-emergency items or services. This includes related costs like mental health care, medical tests, prescription drugs, equipment, and hospital fees.

  • Ensure that your healthcare provider gives you a “Good Faith Estimate” in writing (electronic or print) at least one business day prior to your medical service or item. Your can also ask your healthcare provider, and any other provider you choose, for a “Good Faith Estimate” before you schedule a medical service or item.

  • Save a copy or take a picture of your “Good Faith Estimate,” as you may need to refer to it later.

  • If you receive a bill that is at least $400.00 USD more than your “Good Faith Estimate,” you may dispute the bill.

For questions or to gather more information about your right to a “Good Faith Estimate,” visit http://www.cms.govnosurprises/consumers or call HHS at (800) 368-1019.
